Hi Bill C,not good reading to view your dilemma but it sounds like you and I are a small proportion of the paruresis sufferers that have fell for the double whammy.We have started out with the mental paruresis and over the years it has developed into both a physical and a mental problem,by the very fact the sphincter muscle has enlarged to Arnold Schartzeneger proportions and the bladder wall muscles have lost their elasticity.I know the feelings well,you go into a safe cubical,relax---no stream at all even though you haven't been for 4 hours.You go home having endured the pain and frustration and go straight to the bathroom and even in your own home you relax---no stream,only after a hour has gone can I empty my bladder.Asked a urologist if he could weaken the muscle by surgery but he refused saying it would almost certainly lead to incontinence. But hey ho like most things in life you learn to live with it.
