I think it is definitely a help to have music, or be thinking of a song, or be thinking of anything other than peeing... when you start thinking about it too much, like you said in your first post, thats when you can start to have problems.
People who don't suffer from AP they don't think all day about what they will do when they need to pee. They just notice an urgency, find a toilet and go. Whilst all this is going on they are thinking about work or football or their next holiday or whatever... whereas people with AP are going to work, watching football or planning a holiday and all the while thinking about AP!
You say you are having success in quiet toilets but have you tried busier, bigger places - I find there is less pressure there as there as so many people you are anonymous whereas a in a quiet place if someone comes in it immediately becomes a bit crowded. In the main train stations in London there are normally quite big toilets - I always pop in if I am on route somewhere, even just to stand there and fake it, and I am getting to the point when I can go even if I didn't think i had much urgency...... some times!
