Yes: the security provided by having a catheter with you means your anxiety is significantly reduced; Hence why it can be possible to pee normally in that situation.
When you go for the caths, the nurse will ensure you can use it correctly. You then need to use one at home to prove you can do it on your own. Then you need to "prove" to your subconscious that you can use it in a public toilet by doing so; choose a time and place not far from home, and make sure you are not urgent.
About the "Oh you'll go when your really need to" response. You can print off the FAQ on our website headed "why don't I pee when I want to? what is stopping it?". Then you can give it them to read.
It may be worth knowing that this can hit people who do not have paruresis. Men call it the "boss factor" i.e. they freeze unexpectedly when their boss is with them. Matt Damon reported on a chat show how he froze when President Clinton came in with him! Even Alan Sugar reported on the Jonathan Ross show that he froze when a stranger came up to him at the urinals and started talking to him. Worth telling people this.
