I agree with Tony, a workshop would be of benefit to you. In the meantime, try to be seated on the toilet in a plane. The motion of a plane, or train, makes it very difficult to urinate whilst standing. Many men and women without paruresis struggle on a plane due to the altitude and it's effect on the bladder.
