The purpose is to have someone who knows of your paruresis stood near to you so that when you desense you don't worry what they will think if you can't pee or take a while to pee because they will know why you are having trouble. If you can get used to urinating near a pee buddy then you can go up a hieararchy and try urinating near to strangers because you will be used to someone in close proximity to you. I meet up with other pee buddy's that i met via this message board and we help each other in proximity but also in conversation when we discuss how paruresis has or hasn't affected us since we last met up and this can be encouraging. At home you could use the method of mate standing farthest away from the toilet with the door shut and then increase this by getting him to stand closer to the toilet door and changing the door from locked, unlocked, ajar, and open. It may not be the same as a public toilet but it may help.
