Im glad you've come across this site and thankyou for sharing your experiences.
Im only 25 and was lucky to find this site when I was 23. I used to be severe with this, ie, struggled at home if I had a friend round or waiting outside for me. After getting a serious girlfiend (met online as never went out lol) I improved a bit but it wasnt till I found this site and attending a begginers workshop that I improved significantly.
I wouldnt say that it has completly ruined your life, as you've not let it stop you from going out or attending events etc. I admire you for that as I wouldnt go out atall or not drink from midday so I was empty all day/evening.
Dont think its ever too late to retrain your brain, yes you have become set in your own thoughts about this but it can be treated at whatever age.
As for the workshop, its easwy to think 'no way am I doing that' but you will regret not going and once you have been on one you will wish you did one years ago.
In the meantime, as Andrew sais see your GP and ask to be shown how to self catheterise. It is not unhgeard of for the doctor to disagree with you, but you must insist and remember its your decision. I have used them many times to get me out of bad situations like night clubs and have had no problems what so ever. Definately worth a try at least?
