Breathholding does not work for everyone; when I tried it, like you I ended up gasping for breath; not something one wants to be doing in public!!
I gather from the write-up that one option is, before getting to the point of gasping, release a bit of breath and take a small breath in, and repeat this as necessary. My take on this is that it becomes a distraction technique. You end up so engrossed in monitoring your breath and when you can take another small breath that you cease to pay any attention to the surroundings i.e. cease to be anxious about the surroundings, and hey presto the body pees. Worth another try?
However you say you have a severe case of paruresis. Could you elaborate: where can you and cant you pee?
