The test for the psychological condition of shy bladder is this: if you can pee with no difficulty in what you feel is a "safe" place, but have difficulty when there are people mabout, then it is shy bladder, aka avoidant paruresis.
If this is not hte case i.e. you get no urge but get pain, then it could be a physical problem. For this, it is important that you go to your doctor and tell him/her. You will get referred to a urologist who can check you out . NB a urologist checks for physical problems only; they do not deal with psychological conditions like shy bladder.
It is common to not be shy, but to be self-concious.
It is common also to be OK once the flow has started; it is the getting started that seems to be the main stumbling block.
Having a full bladder but having no urge, can be a sympton of shy bladder.
